What You Need to Know – Client FYI
Make the most of your time at The Center for Counseling and Health Resources, Inc. – for eating disorders , depression , addictions and other issues – by carefully reading the information below regarding appointments, billing and payment, confidentiality, resources and how to get Client Registration Forms.
Appointments and Admissions
You can start the admissions process by calling toll-free 1-888-771-5166 or 1-425-771-5166. Our admissions team will help with every detail.
Changes in counseling appointments at The Center will be accommodated as scheduling permits, though you will be charged the full session fee if you cancel within 24 hours of your appointment time or fail to show for your scheduled appointment. And please note that missed appointments or late cancelled appointments with less than 24 hours notice are not billable to insurance and will be charged to your account in full.
Billing Information and Payment Options
In the event we are submitting claims to your insurance company, you will need to make your “patient portion”" or co-payments. The Center accepts check, cash, Visa, MasterCard, American Express, Discover.
The Center may accept assignment of any insurance benefits. Having applicable health insurance in no way relieves you of financial responsibility for any portion of payment, and any balance owing is your responsibility whether insurance pays or not. Funds received from insurance will be applied directly to the account.
If pre-authorization is a requirement for insurance reimbursement, you may be required to secure initial and subsequent authorizations from the required source, according to the requirement of your insurance. The Center is not responsible for services performed that may be unauthorized or not covered by any third party payer. The Center agrees only to perform professional services that you request and schedule.
If the patient is a dependent child, the parent who brings the child in for treatment is responsible for payment at the time of service.
Billing statements are prepared and sent out monthly. We do not charge interest on accounts, however a re-billing fee of $15 per month will be added to any unpaid balance remaining after 60 days. There will be a $30 service charge for returned checks.
In the event that the insurance carrier rejects your claim, or the amount reimbursed is less than expected, we ask that you make payment arrangements with our Financial Services Department for the balance owing on your account. If the insurance carrier pays more than expected, a reimbursement will be promptly made to you. It is important to notify our office if your insurance carrier changes.
Be sure and inquire about our special Financing Programs and how affordable we can make it for you to participate in your own personalized Intensive Treatment Programs for eating disorders, depression, addictions and other treatment areas. This requires going through a seperate admissions process.
